Print At Home And Save Time...
Found on this page are links to our printable service forms. These include our Massage & Body Treatment Health History Form, our Facial Health History Form, our Nail Services Release Form, Spray Tanning Form, and our Waxing Service Release Form. By printing the forms at home, you can fill them out at your convenience and simply bring them to your appointment. Once you have been to Headlines Salon & Spa, your form(s) are kept on file and can be updated if your information changes.
Massage & Body Treatment Form
This 4-page form is necessary to complete before any massage service or body treatment. The form is designed to obtain all necessary medical and personal information only as it pertains to your service(s). Because massage therapy and body treatments have many contraindications, it is important to be honest and thorough when completing this form. Once brought to us, your information remains on-file and completely confidential.
Facial Health History Form
As with all of our services, Facials are highly involved treatments with many contraindications. It is important to be honest and complete when filling out this form, as well as being sure to indicate any known allergies. Our facial line is marine-based, and allergies to things like seaweed and iodine are extremely important to know before your service. Like all of our forms, your information remains confidential once it is received.
Nail Services Information Form
In order to provide the best services possible, we do require a completed Nail Service Form before we perform any services on your nails. This ensures we are aware of your service expectations and any previous or current conditions that may affect your treatment.
Spray Tanning Services Information Form
As with all of our services, our unique, lay-down tanning application requires us to know some personal information in order to avoid and issues during your service. Please be honest and thorough; this information is completely confidential.
Waxing Services Release Form
Because Body & Facial Waxing are very sensitive services, this waxing form is required before any service is preformed. Answering the questions completely and honestly will ensure your safety as well as that of your technician. All information is confidential.
